The Stress Signature Self-Scan is provided through ScoreApp, operated by Hyper Targeted Marketing Limited trading as ScoreApp, United Kingdom.
When you choose to take the Self-Scan, you leave the Mindful Guide Health website and enter the ScoreApp environment.
ScoreApp may collect information required to provide the assessment, including your email address and your responses to the questionnaire.
Some questionnaire responses may reveal information concerning your health or wellbeing. Health-related information may constitute special-category personal data under Article 9 GDPR.
Where such data are processed for the Self-Scan, we rely on your explicit consent where required by applicable law. You may withdraw that consent for future processing at any time.
The information you provide is used to calculate and deliver your Stress Signature results, identify relevant patterns and provide the information or services you request.
ScoreApp may process personal data on our behalf in accordance with its contractual and data-protection obligations.
ScoreApp is based in the United Kingdom. You should also review the privacy information made available within the ScoreApp assessment environment.
The Self-Scan is an educational and self-reflection tool and is not intended to provide a medical diagnosis.
7. Email communications and Kit
Where you choose to receive emails from Mindful Guide Health, your contact information may be transferred from ScoreApp to Kit, formerly ConvertKit, which we use to manage email communications.
We use Kit to send requested results, educational information, newsletters and other communications to which you have subscribed.
Marketing emails are sent on the basis of your consent. You may withdraw that consent at any time by using the unsubscribe link included in each marketing email.
Transactional or service-related communications may also be sent where necessary to provide a service you have requested.
Kit, Inc. is based in the United States. Where personal data are transferred outside the European Economic Area, appropriate safeguards are used in accordance with applicable data-protection law.
8. Appointment booking through Calendly
We may use Calendly, provided by Calendly, LLC, to allow visitors to arrange appointments or coaching sessions.
